  An "Application for Announced Vacancy and Demographic Information Form" can be obtained from CDOT Center for Human Resource Management (at the address below), any CDOT Maintenance office, any State Agency Personnel office, or by clicking on one of the two links above.

• CDOT Application Form T. (Adobe .PDF format) Application "T" is used only for the Transportation Maintenance Worker I position.  CDOT Application Form T is available at the CDOT Center for Human Resource Management (at the address below), any CDOT Maintenance office, or by clicking on the link above.
