| Achievement Recognition Awards Program Nomination Form NOMINATION CRITERIA AND GUIDELINES The Achievement Awards program recognizes excellence and extraordinary efforts leading to quantifiable or documentable results. The awards focus on outstanding achievement above and beyond expectations within regular job duties, or outstanding achievement resulting from special assignments in addition to normal workload or in the employee's community. All nominations will be evaluated in part on how the nominee(s) exemplify one or more of CDOT's Values: Safety - We work and live safely! We protect human life, preserve property, and put employee safety before production. People - We value our employees! We acknowledge and recognize the skills and abilities of our co-workers, place a high priority on employee safety, and draw strength from our diversity and commitment to equal opportunity. Integrity - We earn Colorado's trust! We are honest and responsible in all that we do and hold ourselves to the highest moral and ethical standards. Customer Service - We satisfy our customers! With a can-do attitude we work together and with others to respond effectively to our customers' needs. Excellence - We are committed to quality! We are leaders and problem solvers, continuously improving our products and services in support of our commitment to provide the best transportation systems for Colorado. Respect - We respect each other! We are kind and civil with everyone, and we act with courage and humility. All categories are open to any CDOT employee or group/team of employees. AWARD CATEGORIES
NOMINATION PROCEDURES CDOT Achievement Award nominations may be made by anyone. While the program is intended primarily for CDOT employees, non-employees who are members of CDOT teams, or outside individuals who have made outstanding contributions to CDOT, may be nominated. Nominations may be made at any time. Nominations will be evaluated quarterly (or as needed) by a five-person Evaluation Committee consisting of three members from the CDOT Employee Council and two at-large members appointed by the Executive Director. The Evaluation Committee may choose to make more than one award in any category, make no awards, shift nominations to other categories, or create special categories for certain nominations. To make a nomination, complete the form above and click on the submit button. Be sure to reference the nominator and nominee names on the cover page. RECEIVING AN ACHIEVEMENT AWARD CDOT Achievement Awards are presented during meetings of the Colorado Transportation Commission. The awards ceremony is conducted by the Executive Director and the nominator, or supervisor, of the recipient. A major benefit of receiving a CDOT Achievement Award is public recognition. The awards are publicized in the INSIDER employee newsletter and news releases are sent to local media in the recipients' home areas.
